Reactivation is a three-step process (please read all instructions before proceeding):

Save (or print) the form before Submitting, then return to this page.

Email the form (if applicable) and your phone number to: gammachipresidentxho@gmail.com with "Chi Eta Phi Reactivation" in the Subject Line. The Basileus will contact you with next steps for reactivation.
